A public inquest beginning next month will examine the circumstances around the 2022 death of a woman outside a local care centre in Moosomin.
According to the Saskatchewan Ministry of Justice, 50-year-old Karen Ireland was brought to the Southeast Integrated Care Centre by the RCMP on Nov. 22, 2022 due to mental health concerns. The ministry said Ireland was placed into a “secure room,” but was allowed to go outside without supervision.

She was last seen sitting in the front foyer at approximately 7 a.m. on November 23, 2022 by hospital staff, the ministry said.
It said that shortly after, staff noticed she was not in her room or outside, and a missing person’s report was filed, before she was found dead outside around 2:30 p.m. about 550 meters from the centre.
In statements issued at the time of her disappearance and death, the RCMP said Ireland was wearing a hospital gown and pyjama pants with no shoes when she was reported missing and noted that foul play was not suspected to be a factor in her death.
An inquest examining the circumstances of Ireland’s death is set to begin on March 30 in Moosomin, with coroner Frederick Kovach presiding.
The ministry said the inquest will take a close look into her death, including the medical cause. The inquest jury will then be given a chance to make recommendations in an effort to prevent similar deaths in the future.
